Basic Fee Schedule:

• NEW PATIENT FOR ADHD  – Without Additional Testing – This includes approximately an hour (or longer if needed) evaluation, review of any previous records you can send me, and, if you need them, reports or letters sent about the assessment.  $1200

  • This includes the diagnostic interview,  and the following options can be added:

    1.  Cognitive testing (limited)–tests of memory, spatial abilities, planning $400

    2.  QB test for ADHD $250 per test.  This is an FDA-cleared objective test for both the diagnosis and treatment management of ADHD.

    3.  Eye tracking for ADHD using the Neurosync device $400 for unlimited testing, for example, on and off medication.  This is an FDA-cleared assessment battery to record and analyze eye movement as an aid to concussion diagnosis, but it also gives some information about attention; it is quicker than the QB test.

• NEW PATIENT FOR ADHD – With Additional Testing (for patients 6 years and older)—This includes the limited computerized neuro-psych testing (tests of memory and processing and/or state-of-the-art eye-tracking testing, which gives objective information about attention and brain function. $2000.

• RETURN VISITS – which also covers any questions or issues, such as prescription refills, that come up between visits. Depending on the condition being treated, return visits are approximately once every two months after the initial visit and then every six months to a year after that. $600

• MIGRAINE EVALUATION AND TREATMENT  – Tics/Tourettes, Other – $800.

• NO-SHOW FEES – A no-show fee of $750 applies to new patients and $400 to returning patients.
  • • Visits may be done via telehealth or in person in Burlingame.
    • Cancellations within 48 hours or failure to attend the appointment will result in a charge of the full price of the visit.
    • I will prepare a “Superbill” you can give your insurance company for reimbursement at the “out of network rate.”

• • Would you like your primary care physician to attend the visit? If you can arrange the time, I would happily have them.
    • $100 additional (per visit)

  • All visit costs will be charged to your credit card at the time of the visit.
